MEMBERSHIP CONTRACT:
The undersigned acknowledges that he is desirous and agrees to pay for the use of the club.  In consideration of the purchase
of this membership the Club does hereby agree to give the "member" the right to use its facilities during the regular hours for the
period of 1 year beginning ______________, 20__.  This membership expires ______________.
ANY HOLDER OF THIS CONSUMER CREDIT CONTRACT IS SUBJECT TO ALL CLAIMS AND DEFENSES WHICH THE
DEBTOR COULD ASSERT AGAINST THE SELLER OF GOODS OR SERVICE OBTAINED PURSUANT HERETO OR WITH
THE PROCEEDS HEREOF.  RECOVERY HEREUNDER BY THE DEBTOR SHALL NOT EXCEED AMOUNTS PAID BY
THE DEBTOR HEREUNDER. THIS NOTICE TAKES PRECEDENCE OVER ALL AGREEMENTS.
